Catered Event Reservation Form

Due 7 days prior to event date

Please choose ONE entree for all of your "Adult Regular Meals"

Please list below any vendors whom you may have personally contracted with. We will need their company name, contact person, phone & email as well the exact service they will be providing They will be a charge of $100 per outside vendor. Depending on the service the vendor is providing there may be additional insurance requirements.

PLEASE READ BELOW AND ACKNOWLEDGE

We will invoice you after your event for the final guarantees that you make on this form providing they meet the minimums on your contract. The only exception to this is if your actual turnout is greater than what is provided here. However, understand that we cook and prepare food according to the guest count provided on this form. Final balance is due 10 days after your event. At any time we can provide you with a tentative invoice.

If you need to increase your reservation counts, please resubmit this form in its entirely.  We cannot decrease your numbers within 7 days preceeding the event. However, we can add small increases throughout the week if you submit regular updates.
